How to prepare
Fast for eight to ten hours before your sample — water only. Take your diabetes medication as normal unless your doctor has specifically told you to delay it, and mention insulin timing when you book so the appointment fits around it. Avoid alcohol the night before. A morning appointment is easiest because most of the fast happens while you sleep.

These two markers answer different questions over different timescales. A fasting glucose measures this morning. An HbA1c summarises the last two to three months. Neither replaces the other, and some of the most useful information lies in whether the two agree.
Two questions, two timescales
Glucose circulating in your blood attaches itself to the haemoglobin inside your red cells. That attachment is slow and effectively permanent for the life of the cell, so the proportion of haemoglobin carrying sugar reflects the average glucose those cells have been exposed to. Red cells live around three months, which sets the window HbA1c covers, although it is weighted towards the most recent weeks rather than being an even average across the whole period.
A fasting glucose does something quite different. After eight to ten hours without food, your body should have brought blood sugar back to its resting level. What that resting level actually is tells you how well the mechanism is working right now, on the morning of the test.
When the two numbers disagree
Disagreement is not an error. Most often it means something changed recently, such as a new diet, a new medication or a stretch of unusual eating that the three-month marker has not yet caught up with. But HbA1c also depends on red cells behaving normally, and it becomes unreliable where they do not: in anaemia, in inherited haemoglobin variants, after significant blood loss or a transfusion, and in pregnancy. Having a same-day glucose beside it is what lets a doctor notice that the HbA1c should not be trusted in your particular case.
What the fast actually requires
Eight to ten hours with nothing but water. Black coffee, juice, sweetened tea and milk in anything all break it, and alcohol the evening before affects the reading too. The practical answer is an early-morning appointment after an ordinary night's sleep, which is straightforward to arrange when the nurse comes to you.
It is worth getting right, because a broken fast costs you half the panel. The HbA1c is unaffected by what you ate yesterday and cannot be spoiled in a day, but the fasting glucose becomes meaningless and has to be repeated.
Why people who feel entirely well are tested
Type 2 diabetes develops over years and produces no reliable symptoms until it is well established. The stage before it, where glucose handling has started to fail but has not crossed the diagnostic line, is where intervention works best and is invisible without a blood test. Risk also concentrates earlier in this region than international guidance assumes, for well-documented reasons: family history, South Asian and Middle Eastern ancestry, weight carried around the middle, and a history of diabetes in pregnancy all bring the sensible screening age forward.
Reading the direction, not just the value
Because HbA1c reflects a three-month window, there is little point repeating it sooner than that. Nothing you do in a fortnight will show. Once you have two or three readings, the trend between them carries more meaning than any single figure, and small movements sit within the normal variation of the measurement itself.
