How to prepare
No fasting is required. Keep eating your normal diet before the sample — removing foods beforehand changes what the panel can show, and is the commonest way this test is wasted. Do not remove gluten in particular, since coeliac testing is only valid while you are still eating it. Continue your usual medication.

Testing two hundred and forty foods at once sounds like it should settle the question. In practice a panel this broad produces a long report, most of which will not be clinically relevant to you — and the value of doing it depends almost entirely on what happens afterwards, with a doctor, against your actual symptoms.
What the panel is measuring
It measures food-specific antibodies: the immune system's recognition of proteins in individual foods. A measurable response tells you that your immune system has encountered that food and reacted to it in the laboratory.
What it does not tell you is whether that food is causing your symptoms. Antibody presence indicates exposure and recognition, not harm, and the gap between those two things is where most misreading of these reports happens.
Why a long list of positives is expected
Reactivity tends to track what you eat most. Staples that appear in your diet daily — a grain, a dairy product, an egg, a fruit you eat every morning — are among the likeliest to come back flagged, precisely because your immune system has met them most often.
That is why the raw list is not a diet plan. Removing everything marked on a two-hundred-and-forty-food report would leave most people with an unbalanced diet, no clearer idea of what was actually responsible, and no way to test the theory. The useful output is a short list of suspects chosen with a clinician, not the whole report.
This is not an allergy test
True food allergy is a different mechanism and a different clinical problem. It produces fast reactions — swelling of the lips or throat, hives, vomiting, breathing difficulty, usually within minutes — and it can be dangerous.
If that describes anything that has happened to you, this is not the test you need, and a result here should never be used to reassure yourself about it. Immediate reactions need proper allergy assessment, and this panel neither diagnoses nor excludes them.
The answer comes from reintroduction, not the report
The method that actually produces evidence is unglamorous. A small number of suspect foods are removed for a defined period while symptoms are tracked, then reintroduced one at a time with a gap between each, watching for what returns.
Reintroduction is the step people skip and the step that carries the information. Removing several foods at once and feeling better proves very little; putting one back and watching the symptom return is far closer to proof. Keeping a written record of symptoms and meals before and during the process is what makes the exercise interpretable, and doing it with medical supervision is what keeps the diet nutritionally adequate — particularly for children, in pregnancy, or if you already eat a restricted diet.
What it does not diagnose
Coeliac disease has its own specific tests, and those tests are only valid while you are still eating gluten — so do not remove it on the strength of this report before being checked. Lactose intolerance, irritable bowel syndrome and inflammatory bowel disease are likewise diagnosed by other means.
Symptoms such as weight loss, blood in the stool, persistent vomiting, difficulty swallowing or pain that wakes you at night need investigating rather than dieting around. Keep eating your normal diet before the sample, since removing foods first changes what the panel can show. No fasting is needed, blood is collected at your home, hotel or office, results take four to five days, and a doctor reviews them with you.
