How to prepare

Insulin is the first thing to move and the last thing most people measure. Blood sugar can stay entirely normal for years while the pancreas works progressively harder to keep it there, and the standard sugar markers, by design, show you the result of that effort rather than the effort itself.
The marker that moves first
Insulin's job is to move glucose out of the bloodstream and into cells. When cells respond less readily to it, the pancreas compensates by producing more, and for a long while the compensation works: glucose readings stay where they should be. What has changed is the amount of insulin needed to hold them there. Measuring glucose and insulin from the same fasted sample is what makes that visible, because the pair together describes how hard the system is working for the result it is achieving. Neither number carries that on its own.
Why the fast has to be a real one
Insulin is far more sensitive to a broken fast than glucose is. It responds within minutes of eating and stays elevated for hours afterwards, so a small snack or a sweetened coffee before the nurse arrives does not simply blunt the result, it makes it uninterpretable. Eight to ten hours with water only, and an early-morning appointment. It is also worth not making the preceding day unusual in either direction, since a very heavy carbohydrate evening and a sudden day of fasting both distort a fasting insulin considerably more than they distort a fasting glucose.
Who gains most from adding insulin
Adding the third marker is most worthwhile where there is a reason to look earlier than the sugar markers allow: a strong family history of type 2 diabetes, a previous pregnancy affected by gestational diabetes, polycystic ovary syndrome, weight that has settled around the middle and will not shift, fatty liver found incidentally on a scan, or ordinary sugar results alongside symptoms that do not fit them. It is also the more informative choice for anyone who has already been told their glucose is fine and wants to know whether that is effortless or hard won.
A marker that answers back in weeks
HbA1c is a three-month instrument. If you change your diet, your activity or your weight, it will not reflect that for a season, which is a long time to wait to learn whether an effort is working. Fasting insulin and glucose respond considerably sooner. For anyone actively changing something and wanting evidence that it is landing, this panel gives feedback on a timescale that is useful for staying with the change, and that is the main practical reason to choose it over the two-marker version.
What a raised insulin does and does not mean
It is not a diagnosis of diabetes, and on its own it is not a reason to start medication.
